WE ARE PIQUA!  Welcome to Piqua City Schools!  We are excited to include your family as part of our school community!

Piqua City Schools uses a centralized enrollment process.  Enrollment must be completed by a parent or guardian who resides within the Piqua City School District.  The online enrollment must be completed on a laptop, desktop, or tablet.  It is not compatible with a smartphone.

Students entering Kindergarten MUST be 5 years old by August 1 of the school year for which they are being registered.

Back to School

Back-to-school and athletic participation forms for your students for the 2024-2025 school year are now available on FinalForms.

You are required to review and sign your forms once per year and after any update or as additional forms are added.  Forms may be updated at any time throughout the school year. If you need to update your child's bus drop-off or pick-up information after you have completed the Transportation Information section on FinalForms, you may call the transportation office at 937-773-1560 and complete the Google Forms link in the Transportation Information section of FinalForms.

Instructions for Parents to access their FinalForms account are below.


  1. Log in to your account

  2. Locate and click the INCOMPLETE FORMS button for your student(s)

  3. If your 7-12 student plans to participate in a sport, activity, or club, then checkbox for each.  Then, click UPDATE after making your selection.  Selections may be changed until the deadline is reached.

  4. Complete each form and sign your full name (i.e. “Jonathon Smith”) in the parent signature field on each page.  After signing each, click SUBMIT FORM and move to the next form.


  1. When all forms are complete, you will see a “Forms Finished” message


How do I add a student to my account?

    Click  MY STUDENTS.  Then, repeat steps number 3 through number 7 for each additional student.


How do I update information?

  1. Go to:  https://piqua-oh.finalforms.com

  2. Click LOGIN under the Parent Icon.

  1. Click UPDATE FORMS to update information for any student.
