Enrollment Information

Kindergarten Registration for the 2024-2025 School Year is now open!  If you have a child who will be 5 years old on or before August 1st, please click HERE for more information about the registration process.

Please note: You must complete the online enrollment application link below on a desktop, laptop, or another device.   Computers are available at the Piqua Board of Education at 215 Looney Road or one of our five academic buildings.  
To begin, you will need an active email account to create a NEW ACCOUNT which allows you to begin the registration process, save your information, and return at a later date for completion (if necessary). After you have submitted your enrollment application, please bring ALL required documentation to complete the enrollment process for your student.


Please complete the following steps for student registration.

  1. Go to the following link to register for a parent account.  If you already have a parent account for Piqua City School’s FinalForms, please go to the link and select login.

  1. Locate the parent icon and click NEW ACCOUNT, then follow the steps to create your account. If you already have a parent account for Piqua City School’s FinalForms, please select login.


NOTE:  You will receive an email within 2 minutes prompting you to confirm your registration.  If you do not receive an email, then check your spam folder.  If you still cannot locate the FinalForms email, then email [email protected] informing the FinalForms team of the issue.

  1. Check your email for an ACCOUNT CONFIRMATION EMAIL from FinalForms.  Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.

  1. Create your new FinalForms password.  Next, click CONFIRM ACCOUNT.

  2. Click REGISTER STUDENT for your new student at Piqua City Schools.

  3. Complete the online enrollment application.

  4. After you have submitted your enrollment application through FinalForms, please contact the Board of Education at (937)773-4321 or any of the five academic buildings to schedule a time to drop off ALL required documents to complete the enrollment process. (See the required documentation below).

    Please note:  Before your child can begin to attend school, the enrollment application and ALL required documentation must be submitted!

Required documentation:

  1. Original Birth Certificate or Passport 

  2. Immunization Records 

  3. Proof of Residency (gas, electric, or landline phone bill). Please note that you must be currently residing in the Piqua City School district to enroll your student.  All new Piqua City School District students must provide the proper documentation to establish proof of residency. The name and address on these documents must match the name and address of the custodial parent or legal guardian of the student(s) being registered.  

  4. Parent/Guardian's Driver's License or Passport (Photo ID)

  5. Current IEP - ETR - MFE or 504 (if applicable)  

  6. Custody Papers (if applicable)

    If you are living in the Piqua City School District and have not yet received an electric, gas, landline phone bill, or MONTHLY utility bill, you can provide your signed lease agreement or closing paperwork.

We are excited to announce that the Piqua City School District has partnered with FinalForms, an online forms and data management service.  FinalForms allows you to complete and sign academic and athletic forms for your students.  This service will replace the “OneView Parent Portal” for online forms.

All current parents within the District will receive a confirmation email from FinalForms the week of June 12, 2023.  The email will contain the necessary information to create your account and complete your forms for the upcoming school year.  If you did not receive or can not find your email, contact your student’s school building or the Board of Education office to have the email resent. If you are a parent of a student in grades 7-12 and already have a FinalForms account, you will not need to create a new account.

Students in grades 7-12 who will be participating in a sport, club, or activity, will also receive an email to complete their forms for the school year AFTER the parent forms are completed.  Please advise your student(s) to check their school email AFTER the parent forms are completed.

Once you have received your email (an ACCOUNT CONFIRMATION EMAIL from FinalForms),

  1. Open and click CONFIRM YOUR ACCOUNT in the email text.

  2. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.

  3. Your account will be confirmed and you will be logged in.

  4. Locate and click the INCOMPLETE FORMS button for the student you wish to register.

  5. If your 7-12 student plans to participate in a sport, activity, or club, then checkbox for each.  Then, click UPDATE after making your selection.  Selections may be changed until the deadline is reached.

  6. Complete each form and sign your full name (i.e. “Jonathon Smith”) in the parent signature field on each page.  After signing each, click SUBMIT FORM and move to the next form.

  1. When all forms are complete, you will see a “Forms Finished” message

How do I add a student to my account?

    Click  MY STUDENTS.  Then, repeat steps number 3 through number 7 for each additional student.

How do I update information?

  1. Go to:

  1. Click UPDATE FORMS to update information for any student.
