We are excited to announce that the Piqua City School District has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign academic and athletic forms for your students. This service will replace the “OneView Parent Portal” for online forms.
All current parents within the District will receive a confirmation email from FinalForms the week of June 12, 2023. The email will contain the necessary information to create your account and complete your forms for the upcoming school year. If you did not receive or can not find your email, contact your student’s school building or the Board of Education office to have the email resent. If you are a parent of a student in grades 7-12 and already have a FinalForms account, you will not need to create a new account.
Students in grades 7-12 who will be participating in a sport, club, or activity, will also receive an email to complete their forms for the school year AFTER the parent forms are completed. Please advise your student(s) to check their school email AFTER the parent forms are completed.

Once you have received your email (an ACCOUNT CONFIRMATION EMAIL from FinalForms),
Open and click CONFIRM YOUR ACCOUNT in the email text.
Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
Your account will be confirmed and you will be logged in.
Locate and click the INCOMPLETE FORMS button for the student you wish to register.
If your 7-12 student plans to participate in a sport, activity, or club, then checkbox for each. Then, click UPDATE after making your selection. Selections may be changed until the deadline is reached.
Complete each form and sign your full name (i.e. “Jonathon Smith”) in the parent signature field on each page. After signing each, click SUBMIT FORM and move to the next form.
7. When all forms are complete, you will see a “Forms Finished” message
How do I add a student to my account?
Click MY STUDENTS. Then, repeat steps number 3 through number 7 for each additional student.
How do I update information?
Go to: https://piqua-oh.finalforms.com
Click LOGIN under the Parent Icon.
Click UPDATE FORMS to update information for any student.
