  • Please fill out the 2026-2027 Transportation Plan Form if your student is needing transportation to and/or from school for the upcoming school year. To be ready with transportation for the first day of school the form must be completed by August 1, 2026. We look forward to transporting your child to school.

  • If your student lives in Piqua City School district and is attending Christian Academy Schools of Sidney, Lehman Catholic High School, Piqua Catholic, Saint Boniface or Saint Patrick, you will need to fill out this form by July 25, 2026.
