Back-to-school and athletic participation forms for your students for the 2026-2027 school year are now available on FinalForms.

 

You are required to review and sign your forms annually and provide updates to any changes in your child's information, including health, address changes, parent contact information, or when additional forms are added.  Forms may be updated at any time throughout the school year. If you need to update your child's bus drop-off or pick-up information after you have completed the Transportation Information section on FinalForms, you may call the transportation office at 937-773-1560 and complete the Google Forms link in the Transportation Information section of FinalForms.

 

Instructions for Parents to access their FinalForms account are below.

  1. Log in to your account

  2. Locate and click the INCOMPLETE FORMS button for your student(s)

  3. If your 7-12 student plans to participate in a sport, activity, or club, then checkbox for each.  Then, click UPDATE after making your selection.  Selections may be changed until the deadline is reached.

  4. Complete each form and sign your full name (i.e. “Jonathon Smith”) in the parent signature field on each page.  After signing each, click SUBMIT FORM and move to the next form.

  1. When all forms are complete, you will see a “Forms Finished” message   

       

How do I update information?

  1. Go to:  Final Forms Login

  2. Click LOGIN under the Parent Icon.

  3. Click UPDATE FORMS to update information for any student.
