Kindergarten Registration is now OPEN.

We are so excited to welcome the Class of 2039!

Kindergarten Registration

  • Do you have a child that turns 5 years of age on or before the first day of school (September 8, 2026)?
  • Please visit https://piqua-oh.finalforms.com/ to complete the registration forms.
 

Registration Process

  • Complete Final Forms.
  • Fill out the Transportation Request Google Form: https://forms.gle/pgHxvd9mB1vVvDu38
  • Schedule your registration appointment: 937-773-4321.
  • Sign-up for Kindergarten screening appointment.
  • Have a successful year!

What do you need to bring to your appointment?

  • Child's birth certificate
  • Parent/Guardian state-issed identification
  • Proof of Residency: Current utility bill, mortage statement, or rental agreement
  • Court-stamped custody documentation
  • Child's immunization record
  • Child's physical record
 

Important Dates

  • August 1: Final Day to submit transportation requests.
  • August 11-13: Kindergarten screening by appointment.
  • September 3: Save the Date - Indian Days (Open house) 5:00 p.m. -7:00 p.m.
  • September 8: First day of school. Have the best year!
